GENERAL LIABILITY WAIVER

& INFORMED CONSENT

THE MEDICINE BAR
WAIVER OF LIABILITY AND INFORMED CONSENT AGREEMENT

Effective as of Sept 4, 2025

By signing this form, I acknowledge and agree to the following:

1. VOLUNTARY PARTICIPATION

I voluntarily choose to participate in activities, classes, events, or services provided by The Medicine Bar, including but not limited to:

  • Yoga and movement practices
  • Sound healing, energy work, or holistic wellness sessions
  • Consumption of herbal elixirs, smoothies, and wellness products
  • Workshops, live events, and retreats
  • Indigenous practices (kambo, sananga, rapé, etc.)

2. ASSUMPTION OF RISK

I acknowledge that participation may involve inherent risks including but not limited to physical injury, emotional discomfort, allergic reactions, or adverse effects from ingredients. I freely assume these risks.

3. RELEASE OF LIABILITY

I, on behalf of myself, my heirs, and legal representatives, hereby release and hold harmless The Medicine Bar LLC, its owners, staff, independent contractors, partners, and affiliates from any and all claims or liabilities arising out of my participation.

4. MEDICAL DISCLAIMER

I understand that The Medicine Bar does not provide medical diagnosis or treatment. None of the activities or services are intended to substitute for professional medical advice. I affirm I am physically able to participate and will consult a licensed medical provider if unsure.

5. PHOTO/VIDEO RELEASE (Optional – include only if you intend to film or photograph sessions)

I grant permission for The Medicine Bar to use photographs or videos of me for promotional or educational purposes without compensation.

6. AGREEMENT TO TERMS

I have read this waiver, fully understand its content, and agree to be bound by its terms. I sign it freely and voluntarily.
